This Passive Balun will allow you to transmit power and video and audio over CAT5 cable up to 1,000 ft. Just use standard RJ-45 connectors on the ends of the CAT5 cable. These baluns have RJ-45 plugs built right in. Model No. 15-U1010AB-2.
